I have issues with the smoke from the train, given you still get hurt while in an 'air tight' hazmat suit.
If it was heat or chemicals, it should say so. But also what good is a hazmat suit if it can't protect from hazardous chemicals?
The simple answer is that it is a death wall to make you go down into the train.
it's anomalous smoke.
If it is anomalous, then why can't we bottle it or something and use it!?
Alternatively, I've also jumped off a roof in Flathill where it breaks both my legs, then use 2 splints and a couple healing syringes to level my medical at the same time. Fortitude gains are slower, but it's less monotonous.
You can get some neat bonuses from leveling some skills.
Not sure about the non-combat related skills, though.
