I went through the portal at C-Block and also managed to go through the Rise rooftop portal, but there was only a locked door and a zip-line that leads back to the lab area.

My guess was that you are supposed to get there through portal, why would the portal be a thing otherwise. If the doors are locked from the portal, you can go to containment block Dirac, and when entering Inner Wing, you will see "Holmholtz control" across the gap. You can just build the bridge over the gap.

Oh wow, I didn't even notice you could build bridges, I was looking for a way into that vent from the other side.

Here's what we did, spoilers, obviously...

1] Complete the puzzle in the Laser Lab and break the purple portal... Thing?
2] Enter the portal at the Dark Lens Containment (by the big sealed door to Security).
3] Complete RISE
4] On the RISE rooftop, head to the helicopter, the portal will take you to Holmholtz Control Center.

To complete RISE:

Navigate around the outer ring until you enter the inner ring, being aware of the snipers at the center and shotgunners around every other corner. Activate a button in the central conference room to open a blast door to an elevator. Proceed up this elevator to the rooftop and quickly run for cover to the LEFT, a machine gun will open fire as soon as the door opens. Clear the rooftop and head to the helipad, there is the portal.
