Can this be achieved by rescuing 50 unique survivors through multiple playthroughs or does it need to be a single playthrough? Can I skip the main questline and just focus on rescues?

Needs to be done in one playthrough. The only survivor you rescue during the main questline is Isabela, so it can be safely ignored as there's 48 other survivors, not counting Brad, Jessie and Otis who also count as survivors.

There are exactly 48 savable survivors in the game.

If you rescue every single one it should be possible to get Saint because main characters who get out alive count too.

Yes, and the best case to fail is The Last Resort.

Generally speaking, rescuing everyone and completing Overtime mode can also earn achievement.
