For some reason. the "find a gun" discussion never happens when I enter the dining room and turn off the radio. Since that doesnt happen, I cannot continue the game, Every time I go upstairs, the barb wire is still there.

Are you referring to the barbed wire that has the deer head behind it?

This just happened to me as well. When I went into the room with all the hospital beds (after the brothel memory), I don't get the conversation about a gun. When I go into the adjacent room with the barbed wire and the piano, I don't find a note on the piano, and when I go upstairs there is no door at the top of the stairs by which to proceed. My sixth sense is stuck pointing me to the door connecting the piano/barbed wire room and the room with the hospital beds. I can get to the top of the stairs in the room with the piano, but it's just sandbags all the way across until the note that says look behind you. I see the deer head behind the barbed wire, but no doors by which to progress (and no look behind you achievement).

So, I see that the game has been significantly patched, and it's possible some of the progress I'm seeing on walkthrough videos is out of date. I exited the game and rebooted it. This time, when I read the note on the sand bags and it said behind you, the little deer statue vanished and I could progress. Not sure why it wouldn't do that before. I still missed a number of items and achievements due to bugs and the game autosaving past the point that I could go back and get them. Must have somehow fouled up the game's triggers for the next events, but it looks like I'm moving forward now.
