No. You just get the occasional dialog from Niko about blacking out, so to speak. But no harm comes from it. (not in the Steam version anyway)

In the freeware version you literally kill Niko.

However, To get the OneShot achivement in Steam you must save only using the beds, not closing the game window unless required to by the game. It's easy to get on subsequent playthoughs though, once you know all the puzzles it can be played through in around an hour.
