you do unlock a higher difficulty after the first run but you get your replay value through persisntant unlockables you can mess with from the start.
Google them. They are quite extensive and very fun.
I wish there was a "Classic" game mode, which would be exactly like Normal, but with no Autosaving and Cassette tapes for limited saves... That would be the most interesting experience for me.
For example.. I did start Not a Hero in Professional difficulty after finishing in normal and surprise.. you only have a knife. This is just dumb as Chris is a veteran and there is no reason for him to go down the mines without being well equipped. This type of unnatural challenge that breaks the immersion for the sake of video gamey thing, turns me off.
I'm not a fan of unnatural challenges that breaks the immersion of sake of trying to make it harder. The only thing I really like is limited saves because it really adds to the strategy required and tension of planning your next moves and taking risks.
I didn't like Madhouse changes overall as they do damage the overall design and immersion by shuffling some sequence of events that ruins the flow.
I did start Not a Hero on Professional and surprise.. you only have a knife.. Which is straight up dumb as Chris is smarter than that and there would be no reason for him to go down the mines only with a knife. So it really ruins the immersion of sake of making things harder.
Get the idea?
There are other ways to make the game harder without ruining it's own design. Maybe more enemies.. maybe enemies are more resistant... maybe the enemies regrow blown up limbs... maybe you die with a single hit unless you block... etc...
Just saying.
In any case.. I didn't find Madhouse harder than the main game.. only rebalanced and different.. some areas were harder, others easier... I think there are even less enemies to shoot to compensate for the fact they generally have more health.
