Most pre-orders gave out the Survival Pack, which was some Large First Aids with the Defense Coin. The other coins are from the other packs.

It doesn't make massive ammounts of difference, moreso if you tok the time to shoot all the american footy bobbleheads and got the other defense items. I think it just adds another increment of defense again. Which only was relevant in madhouse mode.
Yeah, thats all the Defense coin does, but the other coins each do have similar effects, eg Reload faster, get an Attack boost or an increase stagger boost. Plus the coin that boosts Attack, Defense and Stagger together. Of course 5 item slots for 5 coins in certainly a sacrifice.
Not to mention that you can use the defense coin in end of zoe, that's kinda relevant.
Totally with you the item management. I find that ditching the defense coin for trusty old albert-01R helps a lot for saving ammo on jack and margerite. so long as you have both secrets and essence of defence in the inventory, you can take some punishment. Mutant jack hits hard as hell though. i had to horde meds for him.
