Aim for the head, use as many charged shots as you can, and block A LOT, out-running them just doesn't seem to work for me on Madhouse

take cover, run on thread mills, shoot them in the face with a M21 shotgun or a good handgun with upgraded ammo, bomb their way around corners and detonate them.

or just lure them to your entry point, use the thread mill to evade them and flee by climbing the ladder.

I think he is confusing a part near the end where there is two of these enemies and treadmill mining things also, rather than the barn.
There's plenty of ammo leading up to this part, I just used the double barrel shotty and enhanced ammo with the m19 handgun that you can acquire and repair in this section.

there was only one of these guys in the barn.

I saved the magnum ammo from the whole game on Madhouse just for those two, if you can, use that, takes less than 5 shots on each of them

On regular/easy there is 1, on madhouse there is 2.

I used the over/under shotty, enhanced pistol ammo with Wesker's gun and I might've had a flame round from the grenade launcher. I used the bail or hay and crouching and other objects to block the puke from hitting me. Another good way is when they are coming out of the elevator is to bait them into puking, then moving out of line of sight so you don't get hit then lining up a few headshots just around the corner.

Ah, that must be where the confusion is coming from. Haven't played normal yet, wasn't aware there was only 1 enemy there on non-MH.

OP maybe do some re-runs and revisit all the areas you've been to up to this point whilst under the effects of Pychostimulants to maybe seek out and find any potential resources you may have missed. I found some sneaky coins this way. Consider dumping any and all remaining gunpoweder you have on regular ammo to make super rounds also. Dunno what else to recommend. If you can buy or find some of the steroid items then they actually function as Full heal recovery items along with the max HP boost, so I generally hold onto and use them for boss battles/tough fights.
I took them down fairly easy with circular saw on the stair, prior to that I put 3 headshots with double-barrel (2 into one, and 1 into second).

It was given for second walkthrough (I think speedrun under 4 hours is required).
