CPU teams are pretty good about fielding. I've never seen 2 errors in an entire game from them. This achievement probably will never happen. I've never seen it either.
Surprisingly, overthrows by the catcher on stolen base attempts that go into the OF and result in the runner advancing an extra base also don't get credited as errors, which seems like an oversight. I don't know if I've ever seen an error on a SB attempt to be honest.
Also, due to an injury or sometimes seeming to get carried away with pinch hitting moves, now and then the CPU winds up with a fielder playing away from their primary position.
There are people on the OOTP forums that have played the games for many years and hundreds or thousands of hours and have never had a no-hitter by their team, too.
Or just ask the Padres.
So yeah, I can confirm that this achievement is possible. Just need a few breaks go your way.
So basically, dropping the ball or missing a throw isn't an error by itself. What it means is that the SCORER (in this case the game) must have judged that if the defender didn't mess up, he would have gotten an out.
So you basically you just have to luck out to get this.
Funnily enough, I just got this one about a week after writing this.
Seconding this. A scorer's decision addition to the UI would be most welcome
