I'm getting the feeling you're not American. I don't mean that as an insult, by the way. "Big Mac" is the name of McDonald's signature burger, for one thing. It's also sometimes used as a sort of underhanded insult. The implication here being that Makoto is actually "small" or unimportant.

It's just an adaptation for English version. I think it was just "Makkun" in Japanese, maybe I'm wrong.

I'd assume it's just making fun of the fact that Naegi is increadibly small compared to the other guys and even girls. Chihiro is the only one smaller than him iirc. Also what other people have mentioned with Makoto=Mac

I think it also has something to do with that fact that Genocider Jill likes to compare killing to cooking.

Her nicknames are just barely related to whoever she's talking to and it's pretty random. If you hang out with her she seems to have a different nickname for Makoto each time you talk to her.
