As for the request, there are mods for that. It will most likely never be implemented by the dev. The farmers identity plays such little role so there wouldn't be much point to begin with. Thankfully ConcernedApe doesn't seem to be the kind of developer who does things just because it is viewed as the "correct" thing to do.
Human biology is a lot more complex than just make and female. There are intersex people, we just tend not to hear about them very often. It's not that uncommon for people to have atavistic traits where some bit of normally dormant DNA from some ancient ancestor activates and people may grow a small tail or horns. A lot of times these things are simply removed shortly after birth and unless the parents tell the child, they're never even aware of it.
At the end of the day, however, it's just a desire for people to feel like they are being represented when they play a game. What's wrong with that? Why can't we just follow the basic rule most of us were taught as young children and simply treat others the way we want to be treated? Stardew is a game about building friendships and a sense of community. It's hard to be friends with someone if you stubbornly refuse to accept them for who they are.
Also, biology doesn't mention much on "hormone baths", but on chromosomes and how those combine correctly, or wrongly.
Last but not least, people should learn to respect the developer(s) of any given game, do not cause trouble for them, do not demand things, appreciate dev effort/work/vision and not feel entitled. Besides, there is already gay marriage in-game; who says anybody refuses to accept somebody for whom they (think they) are?
Everyone else just really lacks attention and surf on the current trend to try and get some.
Plus I also think he knows what so many others know, that once you open one door, the same people will start asking (or demanding) that you open more doors. At the end of the day you are sitting there with a game that has a totally new vibe to it. And not necessarily for the better. I just don't want to see Stardew Valley becoming a political tool like so many other titles do in the gaming industry these days. Stardew Valley is progressive and well balanced as it is. If one wants to take it further, and implement things that has basically nothing to do with the world in the game, then there are mods, like mentioned.
No, I shouldn't because
1) No one forced me to play it
2) Plenty of other games with gay characters
3) I am not the director of the game and
4) As long as there isn't any material present in the game that would suggest/indicate that the dev/s are homophobes, there is no need to jump to conclusions about them being that.
We can't expect everyone adding in stuff just for the sake of it. I mean a lot of developers does this, obviously. But it is unfortunately not very productive and often leads to a lot of irritation rather than acceptance. Some creators wants to weave things into a game in a more progressive way, where it holds meaning and is relevant.
The wonderful thing about Stardew Valley is that you have a very good opportunity to write your character as you see fit even without specific and selectable pronouns. Plus a thriving modding community where you can find pretty much a bit of everything.

Ultimately this kind of comes down to the OP making a simple request, then some people deciding they didn't like that request and having to drag their grievance politics into it instead of simply moving along to another post. Honestly, if I were the mod for these forums, I'd probably remove every single post except for the one that has the link to a mod that attempts to make dialog in the game more gender neutral. That directly addresses the OP's comment. Maybe it's not exactly what they wanted, but it largely achieves the end result they desire.
Personally, I think it would be good if Ape and crew went through the dialog and changed it to be more gender neutral. It would have a lot of little benefits as far as making translation work easier since they wouldn't need two sets of dialog. Is Ape in any way obligated to do so? Not even a little bit. If I decide this is a hill I'm willing to die on, I can either never play Stardew again or I can go make my own farm sim game that competes with Stardew and I can make sure all the dialog is completely gender neutral. I've literally seen links for "build your own stardew valley clone using python" type projects.
At the end of the day, while I don't identify as LGBTQ+ or anything like that, I just don't see why there's such a big fuss about calling people by their preferred name. It just seems like basic common courtesy. If you meet someone whose name is "Robert" but they say they prefer to go by "Bob" then how difficult is it for me to call them "Bob"? People put significantly more effort into trying to avoid this basic courtesy than it would ever take to simply acquiesce.
