AFAIK, kids happen by themselves after a while, if you have the nursery.
If your spouse is the same gender as you, you can adopt I believe.

I know it's not a requirement, because I never needed to do that, but it might have given you the amount of "hearts" you were missing somehow. Or it was just a coincidence. Might be that it somehow improves the odds in some way.
