I gave 2 gifts before her birthday, was at 8 Hearts. (2 X's in Gift Slots)
Bought a Bouquet, Gave that on her birthday (still 8 hearts) and 2 X's
Thought it was odd there was no mention of the birthday, so Brought a Chocolate Cake
And Bam, let me gift that too!
4 gifts in 1 week, brought me instantly from 8 to 10 hearts in the last gift, and now counter only says 1 X.
Weird xD?
Not weird; I'm not sure the bouquet counts as a gift. Rather, it is a sign in the game that the character is no longer "single" and is going steady with you. Therefore, I don't think it really counted as a gift. That's just my hypothesis, but you get a unique set of dialogue when you give a bachelor/bachelorette the bouquet. So it would make sense this doesn't count as a true "gift."

It says: "As we mentioned above, you can give each villager up to two gifts per week. If you’ve already given somebody two gifts, but suddenly his or her birthday comes, you are able to give an extra gift. Then the checkbox of this villager will disappear and you won’t be able to give more presents during this week."
