kinda difficult since you can change the main characters looks so much.
well, Grandpa's look is "hard coded" and doesn't change regardless of what the player looks like, so why should the player's parents be any different? For that matter, your in-game children won't share your traits either as far as I've seen; so that green skin is probably just a super-rare recessive gene lol.
Point is, I don't see any issue with having a set of generic parents added for everyone; and having them show up at key events like your wedding or possibly your birthday (which should totally be added as a feature)
