It's the same as far as the marriage process goes, keep shoving gifts in their face until they like you, give them a bouquet, some more gifts, the amulet, and you're set, I haven't actually tried it but I assume it's more or less the same for having kids, they'll just randomly ask you at some point if you want to have kids and then one appears a while later.
Granted, that may just be weird luck.
I happen to really like how this game takes things from other games that are usually more complicated and simplifies them while making them still feel complete.
I married Sebastian as a boy and he (or others, for that matter) have never once acknowledged the fact that we're gay.
Also, the only thing you need to have or adopt kids is a spouse and the largest house. Every morning (assumning you don't have two kids) there's a 5% chance that your spouse will ask you if you want kids.
