there is. You have to replace the Stardew Valley save file of your new PC by the save file of your old PC.
Just browse in your old pc to this direction: C:\Users\Username\AppData\StardewValley\Saves\. (AppData is a hidden direction, make sure to display hidden data). In Saves there should be a direction like "PlayerName_1234567". At the same direction of you new computer, you should find a direction with the quite the same name. Make a backup of that direction if anything goes wrong, then delete it from the \Saves\-Direction. Now, go back to you old computer and copy direction "PlayerName_1234567" to the same direction of your new computer(...\StardewValley\Saves\). It's very easy. When you start SV for the next time, you might be able to load your old memory state.
