But you know what they say, if you want to love somebody you need to first love yourself... or something like that.
I guess if the NPCs sent you nice presents, there'd be a "birthday meta" or something ridiculous.
On the other hand, if you got a gift from every NPC that you were friends with, then yes. Birthdays would be a BIG deal, and there would be even more pressure to maximize friendship with the villagers ASAP and possibly to even set your birthday to later in the year so you have time to build up those friendships.
But the level of abuse and the level of meta shift would depend entirely on what a player birthday meant, and there's no reason it needs to be more than the NPC's saying "Oh, it's your birthday today, isn't it?" and maybe a gift that your spouse had a 20% chance of giving you, anyway. (This number has been estimated on the spot and is probably not an accurate reflection of the chance of getting a gift from your spouse. Probably.)
