I've been married to Sam for like a month but I can't get the question for getting pregnant... He have 12 hearts and I gave him the flower bouquet once again but still nothing happens? Is it supposed to be this difficult to get a child in this game? I do have the nursery as well

You need the Nursery (second house upgrade) in order to have a child. Sleeping before 10pm is said to help boost the chance of having a child as well.

If I remember correctly there's a 5% each night that he/she will ask if you want to have a child. I think it was something like that.

You need to be in bed before your spouse, so be in bed by around 10pm, then there is a chance (like 5%) that they will ask about having kids. Also giving them a bouquet is supposed to help.
