A heart takes 250 points to fill.
Even a "loved" gift only gives 80 points, and Alex doesn't "love" Pizza or Salad, he only "likes" them. A "liked" gift gives 45 points.
Read that page on Friendship, you'll know exactly what's happening.
The system just goes more slowly than you think. Two gifts don't equal two hearts. It takes 5 or 6 "liked" gifts to get one heart.
You don't need to know their "loved" gift, but it does make it about 25% faster. The most important thing is to talk to them every day you see them, and most importantly, give them something they like or love on their birthday.
Alex is one of the toughest ones if you're not persistent. He's always hanging out by his house, so you can easily talk to him, but his "loved" gifts are all recipes that take a long time to unlock. Your best bet is to find something he likes and visit him a few times a week.
