I stealth killed all of them (making sure to quick load if spotted) and got the good outcome.
1.) Sneaking out of the church unnoticed.
2.) Stealing the boat unnoticed.
3.) Get to the cult leader unnoticed.
Aside from the caged fanatic near the guitar that you saved, there are others in the map that you can save - the moral points from these may or may not be required to save Duke.
There are 2 caged fanatics inside a building with several bandits and a dog.. quite close (perhaps just north) to Duke's safehouse. You find a weapon attachment in here I believe. And one of the fanatics gives you a key to elsewhere.
I believe there is one other fanatic in a bandit outpost.. near the transmission tower I think?

But I daresay if you at least free those 2 caged fanatics as well as the guitar fella and the 3 main points above, you should be in the clear.
♥♥♥♥ beans, I think this is the kicker. I shot one who I didn't realize had surrendered at the tower. He was actually the first bandit I'd seen to surrender and I had no expectations they would cease combat.
I can confirm that you can do all of the other mandatory "good guy" things and still leave Duke to die.
No kidding? My run must have just been chuffed as that's the only non-"good" thing I've done.
Of course, I was speaking for ending of Volga solely. In fact, I killed all surrendering bandits (on top of killing Saul) in Caspian as well, and still got Damir to stay with the crew. Thus I doubt it has any impact on the moral points. Tbh I am really confused with TS' problem, if he did indeed do everything he mentioned and still got the bad ending for Volga something is very off. I remember there was a bug in Last Light when certain data carried through autosave even though the chapter was restarted, perhaps there is a similar case. Doubtful, however.

I honesty don't know. Maybe it was exploring the whole map and the interactions with the locals. The 2 fishermen who talk ♥♥♥♥, the father and son who had the radio and those crazy guys who survived the "electric demon".
you could just download a save file from the internet where duke is safe, pretty sure it isn't hard to find
