Karma
Sometimes i'm forced into fighting situations by the story. Does killing in those sequences (like the one at the start of the game where you have to steal the train) cause bad karma?

Not entirely sure about that beginning area to be honest. Generally any point you don’t have a choice in fighting you are allowed to go all out. But as long as you don’t murder the occasional surrendering bandits, and sneak past without murdering “innocents” like cultists then you should be fine.

I honestly dont believe it matters but do not kill those that surrender they are no longer a threat just knock them out. Bandits you can kill but people like cultist or slaves for example you do not want to kill even though they will try to kill you. Its better to knock them out or use stealth and avoid them all together.

I don't recall ever being forced to kill humans in the Volga. You can sneak through everything. Essentially, killing bandits that are hostile won't cause negative karma. There are a few bossfights that have no negative karma either.

I remember being pretty ♥♥♥♥♥♥♥ godsmacked when killing the people burning in fire on the desert map tunnel to that house lead to bad karma instead of mercy killing.
I don't know about the first area. The swamp you can kill bandits and monsters but try not to kill the fish people. For the desert area try not to kill slaves, and for the forest you can kill bandits but pretty much nobody else.
