Gotta say, its kind of lame that the game punishes you so much for playing in any way besides super peaceful. Going camp to camp knocking everyone out is super boring and inevitably ends up taking a BUNCH of medkits/chems since youll get noticed sooner or later. And the melee knockout prompt doesnt show up sometimes so you have to stand in front of an enemy thats shooting at you and wait for it to appear, often resulting in getting hit numerous times.
Its like at that point in the game you finally get access to all the great weapons, the Helsing, decked out valve rifle, etc, but you get punished for using them. Oh well, still a good game.
When you get to the overrun at the Forest camp, save the hostages.
Bandits are fair game so kill if you want to.
You can kill ALL the BANDITS and THUGS without bad karma consequences throughout the whole game.
I massacred every single bandit in Volga, Caspian and Taiga bandit camp, always got the good ending without any trouble.

Its a very good decision coming from the devs since its still a first person shooter. It would be awful if we had to spare absolutely everyone.
Plus you won't feel bad at all for stabbing murderers and rapists!
Yes you have to save the bandit thats about to be eaten by the wolf at the beginning of taiga.
Knocking out Saul too instead of killing him, even though I dont get it why it is so since it clearly doesn't change anything, maybe a remnant of unfinished content? who knows.
I know it's from a year ago but maybe you'll feel the need to replay the game - there is a side mission with Saul if you go to bed and sleep through the night (of course, if you don't kill him). I missed it too.
I had a silenced ashot and would always knock out pirates who are in my way. But once, I was sailing to the gate leading out of their camp, I got spotted and had to shoot them in the legs then knock them out while they were dazed in pain. I only had two accidental kills (one shot in the chest and one shot in the groin) but it still prevented Alyosha from being injured and get my happy ending.
so, yeah, if you accidentally kill aggressive kids (at least, 2) you are a-ok.
On the other hand I didn't get "Damir" in the Caspian level while the only "bad" things I did there was that I was unaware of the imprisoned slaves in the oasis (so I couldn't rescue them) and that I knocked out 4 slaves out for my blood at the airfield hangar...
So I suspect that gaining or losing moral points in those levels is paramount in determining what ending you'll get..
...And I never lost any moral point over killing slaves/cilds of the forest shooting at me, only for killing people that surrendered.
