Mental is actually Sam "Mental" Stone, basically a evil doppleganger of Sam, his a multiplayer skin in one of the console ports of the game.

The good Sam is called Sam "Serious" Stone.

Actually that model is called "Mental Mate", but that's not Mental.

Mental looking like Nick Cage however is a reasonable theory.
