[TUTORIAL] How To Play Splitscreen With 2 Monitors
When searching far and wide for a solution how to play Serious Sam 3 splitscreen separated by monitors with my friends who refused to play it on one screen i found it. The answer is Nvidia Surround.

No idea how to do this on AMD's side. Read somewhere that Crimson doesn't support eyeinfinity any more, hopefully someone down in the comments can clear this up.

Requirements: 2 Monitors @ Same resolution(duh)
1 GTX 470 or Higher(Will work with pretty much every GTX card.)
A game that support high resolution(4K and ♥♥♥♥ed up aspect ratios)

1. Open up Nvidia Control Panel and navigate to "Configure Surround,PhysX"
2. Enable the checkbox "Span displays with Surround"
3. Select what Topology for what kind og splitscreen mode you want(Horizontal or vertical).
4. Move the figures displayed as screen to your preference.
5. Apply.
6. Bonus you can Toggle Surround on/off by pressing CTRL+ALT+S if you have keyboard shortcuts enabled.

Serious Sam 3 can support 4 players splitscreen and it should be able to setup this with Nvidia Surround but i haven't tested it.

Now in your game that supports splitscreen you will probably need to select the highest resolution available. Also do remember that using splitscreen requires your GPU to render almost twice as much data so your FPS will take a hit.
