Make a level 1 dungeon monk, create a few low level dungeons (preferably with decent amount of floors) and use an other monk to go into them (to prevent the monk from levelling up.
Every now and then you will see a group of 4-5 items in red-ish colour, those have "negative" titles and can contain failure titles.
If you are at end game, running "small" and "few" low level dungeons with marona only should take very little time per floor, and 1-2 high speed bottlemails can let you bring those failure items back pretty fast too.
It's still random, but after level 40 or 50 it becomes a pain to find failure titles from spawning characters and it becomes really fast to clear those floors (not to mention it can help build a bit more SP for Marona and a few other characters by clearing the few enemies).
Also at lvl 9999 Marona, I find a failure phantom every 3 “selections” on average
