I saved every little sister and still got the bad ending

I don't understand what happened. This is my first playthrough. I rescued every little sister I came across (although I'm pretty sure I missed one or two) but I never harvested one

In the final mission when you have to pretend to be a big daddy and escort one, she died, because the dumb idiot walked point-blank in front of a machine gun and went from full to 0 in like 2 seconds. Was that what caused it? Or did I accidentally harvest one and forget about it? I'm so confused

Each area you enter, if you press pause via controller or escape via PC while in game, it will tell you how many littler sisters are available. Like 0/3 or 0/2. You have to save, not harvest all of the available little sisters. Before you move onto a new location, check to see if you've gotten every sister.
