Bioshock Achievement Brass Balls - Questions
In order to achieve this achievement, the player must finish this game on hard without dying or getting revived by a Vita Chamber. I get it. What I don't get it is.

1. If I die, will the game save "remember" that I died, even if I reload the save to the point where I saved the last time?

2. I accidently got revived by a Vita-Chamber due to a setting bug. If I reload, will the game still remeber that I got revived? Or can I safely reload as much as I want to?

I can't imagine anyone legitimately getting this achievement without having to reload their game repeatedly, unless they use game or achievement cheats.

As far as the accidental revival it's hard to say with 100% certainty but I think as long as you load to before you were revived by the vita-chamber that you should be good. According to an old post on the Xbox forums, it still works as long as you load a game save from before you were revived.

Ofc you are allowed to die. The requirement is to not use Vita-Chambers. Turn them off in the settings. When/if you die, you get sent back to the main menu, simply hit continue or load another save and go on.
If you die while they are still turned on load a save before you got revived and you should probably be good.
