I think it's possible that yes, there are definitely sane individuals in Rapture. Otherwise, it literally makes absolutely no sense in 2 how anything is even working (lights, security systems, DOORS, electricity, etc)
In Bioshock 2 we see a host of sane individuals - Lamb, Sinclair, Grace, Stanley, etc
I don't think any of those guys left to the surface and came back. They must have been around even during the events of 1.
I think that Rapture, even after the Civil War, was still barely trucking by with its ♥♥♥♥♥♥ up populace - you just don't see anyone doing maintenance (except Big Daddies) because they're too busy trying to kill you on sight due to being either mind-raped by pheromones (Ryan, Fontaine used them) or being mind controlled by philosophy (Lamb).
If you slow down a bit a few times in 1 and 2, you can actually hear some scripted events of Splicers doing things rationally (not the correct word, but it's the best way to describe it - they are babbling to themselves and mumbling, but they're still doing a task correctly).
For example in 2, I just now sat outside a door listening to two Splicers arguing over hacking a turret. I also earlier stood behind a pillar and heard a group of Splicers talking about what fate befell Jack from the first game - One of them thought he escaped with a group of Little Sisters, one of them thought he killed everything and left, and another just didn't give a ♥♥♥♥.
So this shows that while the Splicers are indeed insane, they can still reason and work and do things, and some of them are atleast somewhat aware of the events in the passing of time.
It makes me wonder if all Splicers are actually hostile to everyone that isn't a Splicer period, or if it's just them attacking outsiders, and they leave people like Grace or Stanley alone because they know about them.
