Maybe this will help, it's part of my playthrough
I play with the fog on, because 1 I don't have an issue with the draw distance, it works out fine, and without it the game looks bad, imo. 2, Enemies don't have the extra line of sight on you with the fog.
As for ammo, at least through the whole normal playthrough, I had way more than enough ammo at all times. Just don't hang in the same area for too long and keep progressing
if you expect this to be some easy ♥♥♥♥ game you can beat pretty much in one try with no problem then it's not your type of game. there are attacks you simply can't avoid, just deal with it and accept the lost health.
Usually I explore on bit at a time. I've found all the keys so far and managed to stock up on ammo and lives, but lvl 3 is still a nightmare >_<

I'm currently replaying the game on Hardcore (but with the closer fog because I'm not quite ready for the hordes of snipers on steroids), just beat level 3. There's a whole lot of backpacks in the second half or so of it, I think three at least, so I think you can just accept a few deaths, since it doesn't reset any progress but restores your health.
The blowdart guys actually miss very easily if you run sideways (or maybe they are 100% hitscanners and just randomly miss sometimes, I dunno actually). The riflemen seem to be more precise but they are weaker and die of one assault rifle burst. My advice is just keep moving, don't forget about cover or hiding when possible, and since I believe the actual city part (with a lot of small houses) is the hardest part of the level, a specific advice for that part would be to move on rooftops and kill the runners downstairs with the tek arrows. Worked wonders for me.
Also there didn't seem to be that many in the first place. There are probably some I'm forgetting but the only ones who I remember after 2 playthroughs of the level were a couple on the "city" rooftops, one on an elevated ledge to the left in that one place where you go up on the right, go across and then lower a whole bunch of pillars to open the stairs downwards, and one in one of the houses in that same area.
Honestly the only level I remember them bitterly from is level 8. :X

Only after exploring for many hours I have up and decide to leave the level and I finally found out there was a boss. -_-
Haha, yeah I also didn't enter the portal at first, thinking I missed a key somewhere.
Anyway, the main problem with lvl 3 is all the respawning enemies, who make it hard to find the right way. Only thing I missed was a switch to open a new passage. But you have to climb up a tree in order to do that. I just couldn't figure that out while being constantly under fire.
Finished the game last night. :3
Chronoscepter makes the last fight so easy, so I did it again without using it.
But going through the first few levels using cheats, all weapons, unlimited ammo can be a lot of fun too ^^
