Can someone help me with this achievement?!

I have every achievement in the game except for "The Full Truth" and I can't find a guide for it anywhere!

You need to ask every question you can to those weird screens which give you a "vision". Just get to the first of those weird screens and ask any question you have never asked before. Restart the game and do that again until you get the achievements.

But I've already done that, multiple times even, so is it just a glitch?

There are three of these scenes. Each vision has three options. The first one you go to always has the same three questions. The second has a different one instead of what you chose the first time, and the third has different ones instead of the second and first choices. To get to these screens:
1. Upon entering pony island the second time (the first time with the devil heads), the credits will be grayed out (I think). After losing all three of your lives, where the credits button was there are a bunch of #'s. Clicking on these brings up a prompt, asking for the password. The password is Baphomet, and you can find this in the options under password info.
2. In adventure mode, soon after you enter Sin Desert, there is a split in the path. Going north takes you to the Cave of Truths, and you can find the second screen here.
3. In one of the puzzle breaks with the splitter signs, there is a flower at the right of the screen. If you have a butterfly go on top of it, it will open up the final screen.
