Are we sure that the sword pieces aren't bugged?

I am still missing one segment for the final sword, I have been to all the sword points listed on the overview map x5 thinking I missed something and yet all known locations have been found and used. I don't know what to do. The ability to track down the last few things I'm missing is driving me nuts, two butterflies, two skils, and one sword fragment. I do have a heart that I collected yet it never disappears from the overview map as well.

Fwiw, I had the same problem with the last sword piece. It is the only one NOT located on the above ground map. To avoid further spoilers, I won't say where it is, but you can use a guide to tell you where it is.

Near the top of the included comments, there's advice on how to find it in the indicated location. It's obscured, but I slapped my forehead when I finally found it.
