How to use Potions in Battle?

I can't figure out how to use potions during battle, or how to revive fallen companions? Can someone help?

equip them at the start. But can only equip one item, either potions or ...

Doesn't seem to work for me - I gave one dwarf a potion and instead he has grenade icon with no grenades to throw. Seems like a bug...

That happened to me too after the fight at Wackenstein (where you pick up grenades to use). However when I reloaded and manually set each item for all partymembers again it worked in the next fight.

Completing the fight with the golem left one dwarf critically wounded. I have 3 health potions and can equip one before battle but it doesn't show up. Need it to make it to exit, always get killed in the second part of the hall. Bit of a gamebreaker these 3 battles, certainly breaks the story part in my opinion. Any tips except replaying that golem part would be very welcome
