"Domino" kills where you do not initiate the first kill
-NPCs killed by spawned rats/bloodflies
-Killing a Nest Keeper

-Wolf Hounds
-Grave Hounds
-Clockwork Soldiers
-Bloodbriars

Nest Keeper is a person around the nests. They are kind of rare to find, but there are few instances here and there. They act a lot like Weeper from the first game. Basically a zombie.
For the second question, check your stats (menu - stats - mission/playtrough tab; not global!) from time to time. If you have 0 kills and 0 spottings, you're fine to get Clean Hands and Ghost achievements.
I think that found bodies are ok and will not ruin the stealth-based achievements. It didn't in the first game, I have no reason to believe it does in here. As long as you have checked ghost and no kill in the end-mission screen, you're fine. I'm pretty sure about that.
If it was like Dishonored 1 where they didn't then its likely that they don't in Dishonored 2.
