Why the Protag doesn't mention it? I blame dev oversight
The protagonist doesn't mention it because it wouldn't make any sense to do so. They *know* why her arm and eye are there: because the conditions which removed them are no longer true.
This will become important in the Death of the Outsider, so ... yes, saving Stilton from being insane, not allowing him to go to the seance, is canon to the story. She DOES have both her arm and eye, in DotO. If you have not played it, you'll want to pay closer attention to the actual story when you do, because all of this is 100% explained in the games.
There are several complete versions of Stilton's mansion - the version you enter which is run down and gross, which is basically the same as if you just wander in, allow the seance to happen with stilton, and don't kill anyone, and leave. But if you save him, you'll notice that the mansion is undergoing a radical renovation, which had been in progress during the party but was never completed if you allow him to do the seance. There is also the version which exists if you *kill* Stilton, where it was sold, and is being sold a second time, due basically to the haunted areas of it.
This level is a masterpiece of storytelling - the devs did not overlook anything. You have to read and listen to the audiographs available to you, everywhere you find them, to get the details.
Saving Stilton is canon. As for the reasons of the arm and eye in DOTO, the answer is that it's complicated, and a bit of an early spoiler for that game. I thought it was a really clever idea though.
For real: there are very good reasons why you see what you see there.
Also note that the trailer and artwork for the game *are not* necessarily the things you'll see directly IN the game.
