What constitutes being spotted?
Trying to go full stealth for this playthrough. If the red lightning meter isn't full, have I messed up my stealth?

No.
I'll advise you to enable the training tips (in case you do not hear the detection sound). As soon as someone finds you for the first time - a training tip will pop up.
The main thing is for you to know how to neutralize the real Delilah.

Yes and no.
No, because when it is full white/not full red, it doesn't count as detected - it only means that guard saw something and he will only come to see what was that.
Yes, because sometimes the game can bug out and you will be detected immediately, even through walls/doors.

Vlad gave good advice, just turn on tips, so when the tip about detection/fight appears, that will mean you are detected and you are 100% sure about it when it happens.

I'm still getting the "learn about combat" message when I get caught, so I think I'm good.

There are basically four stages of detection.
1. Awareness meter pops up with no color or just slightly white. This means that someone heard something or caught a brief glimpse of something but, it's not enough for them to check out.
2. Awarness meter fills partially in white. You've raised someone's suspicions and they will investigate around your location. A sharp sound will play.
3. Awareness meter begins to fill with red. Suspicions are high and you'ld better take cover quick. You're not detected yet but soon will be if you don't hide.
4. Awareness meter fills in red and it breaks apart. You've been spotted (detected) and npc's will attack. This is always accompanied by a distictive combat sound.

Enemies of course are most sensitive to sitghtings and sounds than non-combatant npc's but civilians can be a source of detection if they see you do something illegal or aggressive. In some cases, they can also turn against you if they witness you stealing something.

One exception to the awareness meter indicating a detection is, when you shoot the head off of a Clockwork soldier, his meter will turn full red and breaks but, this is NOT a detection.

When in doubt, hit ESC and click on "Stats". This will tell you if you've been detected or not.
