Good Doctor: either Non-lethal or Stealth?
Okay, so I found that to get non-lethal solution you need to talk to Vasco, and get the safe code from him.
The problem is, doing so provokes Crown Killer into action - and they have that insta-detection radius around them. It's even outlined if you use Dark Vision. I prepared the antidote and knocked them out via drop-assasination. Normally, it's the fastest way, but Crown Killer STILL detected me mid-fall, ruining my Ghost achievement. If I attempt to blink behind their back, they notice me immediately.
Now, I doubt I can get the antidote without provoking them via cutscene, I can't knock them out since it provokes them anyway, and apparently I can't approach Crown Killer undetected once they are provoked (barring Stop Time maybe?).

Does anyone have bloodless AND stealthy solution to this conundrum?

Nope.. I killed the witch.

I shot her with 5 sleep darts and threw a bottle of Chloroform and she didn't blink.
I was forced to kill her.

The Crown Killer didn't appear to be provoked by my conversation with Vasco, and three sleep darts sent her to Sleepytown.

From my experience , stalk and choke ( in this case , inject the serum ) should do the trick , although I played Emily as the protagonist . Drop attack doesn't work because you cannot drop and inject the serum so you end up falling on the enemy and trigger the alarm . I'd recommend you jump down from a beam , sneak to her back and hit the Ctrl key hard.

She attacks in the cutscene and kills Vasco. You are not detected during cutscene, but she's left provoked. For me this cutscene happens whether or not I talk to her beforehand.

She appears to sense me immediately when I'm in certain radius. I can try again, but I don't have high expectations.

I was able to solve this problem by getting Bend Time. The level has enough runes for that.
I slowed down time, waited for Crown Killer to pass below me, teleported behind their back and took them out. No detection, no kills. Perhaps it's possible to execute this without Bend Time, but it's still helpful.
On a side note, talking to Vasco ALWAYS triggers Crown Killer cutscene for me. Also, they appear to be immune to the sleep darts - at least five of those didn't even slow them down.
Still, it's a bit annoying that I somehow got detected during attempt at drop asssasination. Maybe it was a one-time fluke...

Although you seemed to solve your problem already, I can confirm it's possible to get ghost/non-lethal even without Bend Time as I did just that on my first playthrough. It's necessary to trigger Crown Killer transformation one way or another in order to proceed at all, and I don't think many of those will get you detected anyway, although it seems like that. At least talking to Vasco and being surprised by Crown Killer's sudden attack doesn't. However, it is very much possible to be detected by Crown Killer afterwards, but I've never found it particularly difficult to avoid either.

Also, I'm quite certain it's possible to perform a "drop-inject" because I think I once did just that, though by accident. I had intended to do a non-lethal aerial takedown. :)

I cannot argue much if someone has actually successfully sleep-darted Crown Killer, but I thought it was supposed to be impossible. Also, sleep-darting or any attempt at Dr. Hypatia's life will trigger her transformation into Crown Killer.

And, it's also possible to proceed towards the non-lethal solution without talking to Vasco. You need to find him anyway to get the safe code. I think if you listen to the audiograph in the same room where he is, it will trigger Crown Killer transformation, but she will not rush into that room. Just an alternative approach to try, though you will still have to leave that room and head for Vasco's lab without being detected.

After having said that, couldn't one just sneak up to her "before" she transforms, lets say, just after talking to her, while she's muddling around? Take her down non-lethel and then go talk to Vasco?

No, any attempt to choke/sleep-dart/kill Dr. Hypatia will trigger her transformation.

I found the easiest way to get non-lethal stealth was to use the teleport power to land right behind her. You still need to be quick on the trigger with the syringe, but it only took me two tries (excluding the ones where I landed much too far away).

If you are playing with no powers on a low chaos run, you still have to be sneaky and come up behind her... I used the ledge above the central area to survey the surroundings to then make my move. It worked like a charm.

In any case if you are going for the Souvenirs achievement, you have to deal with her in a non-lethal way else that syringe will not appear in your quarters
