How to optimize the ping in multiplayer?

My friends join my server and we can play normally with a low ping, but when we fight with npcs my friends will be extremely laggy. How can I optimize it, can any addons do work?

Please give me some more information:

• What's yours and your friend's PC specs ?
  
• How many addons do you have ?
  
• What kind of map are you playing ?
  
• What kind of NPC are you spawning ?
  
• What exactly happens ? Does your friend's PC lags or does his ping goes high ? There is a difference between both.

High-Performance doesn't really define anything but alright. I assume you guys have a decent setup.

gm_fork is a really big map with a lot of details, perhaps try another map to spawn NPC's.

Vanilla NPC's (Normal Half-Life 2 NPC's) cannot lag your game at all even if you spawn 50 of them.

You either downloaded a mod which causes that OR your computer is not powerfull enough. But since it's not the case, then there is a mod causing this issue.

Can you list your mods ?

Just some addons that don't consume computer resources a lot, like M9K series, CW, TFA.

If you're playing via P2P, that's why - P2P has very low bandwidth to work with.
You should look into either a more closed-off map (to reduce networking load) or port forwarding (much higher networking load limits).
