Not everything in GMod is online-based. Even on Singleplayer without any addons would you see blood, corpses and other scary stuff and hear the NPCs swear out loud. As a reminder, this was originally a mod for Half-Life 2, a game which was already rated Mature/16+ in 2004, the same year GMod was released.

Gmod is +12 if you're just playing base game and some basic addons and mods and playing multiplayer with friends.
Gmod is +16 if you're downloading some teens and up addons and mods and joining servers with unkown people.
Gmod is +18 if you're downloading "that stuff" from internet and playing in "those servers".

I started playing when I was 7 or 6. Garry's Mod isnt bad unless you do ''bad'' things in it.
