1. FastDL is nice, recommend it.
3. Good staff are important to have on a server, I recommend creating a steam group for your server and allowing users to apply there. The more detailed the questions on the app the better, possibly have a minimum amount of hours (50 for mod, 100 for admin?)(just an example).
4. Like I said, steam groups are the best way to make a server forum.
6. Personal experience for me: Listing some of the key addons of the server in the server title is VERY important.
Hope my answers helped you out.

3. It's mostly a test of time, you may never know if the guys would be nice enough to watch the server, but if friends do understand the rules AND know they mustn't abuse their powers you can go with them for a start.
4. I don't think it really matters unless you reach a 100,000 total players milestone.
5. I don't think so, as for perks, go for cosmetic changes only(that is, giving donors admin/mod privileges is crossing the line).
6. While some might have told you to advertise it in the Gamemodes subforum, its actually not allowed because the default steam forum rules don't allow them. Go for a better way, such as actually serving your players well.
What I have so far:
Pointshop with TF2 hats
MapVote system
24 maps total
ULX
I'm still looking around for a donation system however, I need one that doesn't use MySQL, because I really don't feel like setting one up.
