The entire "gameinfo.txt" file for "Portal: Aperture Deja Vu" is as follows:

If you play other Portal mods in the future, and need to edit the gameinfo.txt file, you should erase only the last part of the file (from "FileSystem" to the end) and then paste the text from my first post above.
Once you update the gameinfo.txt file, and then restart Steam, it will appear in your library again.
