Beat the game once. After that start a new game. Radios will now have a red light, and at least one will spawn in each test chamber. This is in addition to the already existing radios. A few chambers have more than one, and there will be multiple during the escape sequence.

When you have found a radio pick it up, you will notice that you get signal interference as you move around holding the radio, you need to find the location where the interference is at its strongest and hold it there for a bit that will make the light turn green. Make every radio light turn green (26 total) and you get your achievement.
