The voice beyond the grave?

I've played the game about 5 times now, from begining to end, in different ways. How the heck am I supposed to get this achiemevent.

Guides tell me that I'm supposed to "chat with a vision", which is the same description as the achievement, but... where am I supposed to do this?

I got it on my playthrough. It was at the very end right before the ritual. The woman doctor was talking to me and I also had an inner voice called "truth" that would chime in a lot.

Basically I think you have to go fully insane/ occult and get to that part then pick the correct dialogue.

It's strange, I've been there multiple times with different amounts fo sanity/occult, but thanks for the info, at least I know where to look for it.
