Report a translation problem
I think you have to beat the game with a low Occultism level to get the "good" ending. Basically try your hardest to never learn about the cult, or believe in it. Don't read anything about it, do not talk about it.
You see something wierd? Blame it on your constant drinking. Don't believe, don't see it, don't say it.
I am also very sceptic about the in game choices influence especially when you can only chose from lines written in secret letters (how fair choice is that?).
I have no idea how to get it and there aren't really any good guides out there.
Apparently nobody knows exactly what triggers which ending.

I got sarah suicide by doing, no drinking, rejecting leviathan, rejecting every decision, and using dialog choices that support sanity with Dr. colden in ch 14 and with sarah in ch 14
- Learn as much as I could about the mythos ;
- Follow every path that would lead me to the "truth", i.e., constantly risk my sanity ;
- Yet refuse any path that demanded I reject humanity or follow Leviathan just because of reasons or because "it's inevitable" and stuff ;
- Drink every time possible.
I thought that in the end game, my occult knowledge would give me the weapons to fight the threat. I was kinda right, but I didn't the the "good" ending. For that you really have to remain untainted and innocent.
I'll say that the endings make a lot of sense.
