My conclusion is, if you're interested in getting all achievements in one playthrough, make sure you also get to pass the strength test to get to the warehouse via the underground tunnel, since there's a tribe mural to be seen there that's also required for another achievement. Both the letter must spawn and the strength test must be passed. If any of these two fails, you should restart the game (which shouldn't be a problem at that early stage of the game).
You put most points to occultism and medicine at the start? You mean the only two skills you can't put points into, and must be leveled up by collecting and examining things?

Besides, the quote you try to discredit is true. The only way to have 4 Spot Hidden by chapter 2 is by putting all skill points to Spot Hidden at the start of the game. I'm not saying you need it to get all achievements. But there's a letter inside a little room in chapter 2 that I've heard will only spawn if you have 4 Hidden Spot. I'm NOT 100% sure about this, I didn't test this.

I don't own the game but if you check my profile you'll see I have all of the games achievements and so clearly have played it. In this case I played it through library sharing but the game being revoked due to Humble's pricing error would also explain why I don't own the game.
