How to get the"Farewell" achievement?

I got my occult to master level but there is no option for me to refuse ritual. Did I missed anything?Anyone knows that?

The farewell ending involves Sarah. I'm still not sure how you unlock it, I got it on my first playthrough, if I discover something replaying it I'll let you know.
Edit: apparently you need high occultism and high sanity. Good luck on that!

★ Sara's suicide enging. To obtain this ending, I did:
✔ drink alco (I think it doesn't matter, still)
✔ read all unholy books
✔ don't choose answers with R'lyeh language
✔ accept Drake's help + saved Drake
✔ refuse to accept Leviathan's knowledge 
✔ refuse to drink medicine, refuse to eat Leviathan's flesh, refuse to take the gun, etc
✔ spare Bradley

Did the same things except I can confirm alcol doesn't matter, neither does choosing the answers in aklo apparently

Spare Bradley, refuse leviathan, and when talking to dr colden at end. Always be the "same or refuse options" same with sarah

I got this achievement just now,no need to read unholy books,others is right,thanks for your helping. XD

I ended up getting Farewell first playthrough, but:

I didn't drink alcohol
I didn't read any books
I chose SOME answers with r'lyeh
accepted and saved Drake
refused knowledge and all the "tests"
spared bradley

So it's probably something to do with your sanity level if you can get it or not.

okay so far the only thing i can see that is different in endings at least for farewell is bradley and leviathan ♥♥♥♥. so we need to know what those 2 events do to get nameless cults

Going back and loading my save right before the final choice, I was also able to get the Call of Cthulhu ending and counter-ritual ending under the exact same circumstances. Also for the record, I leveled Investigation to max and Spot Hidden in that order through the game, didn't really touch anything else.

I think you need to keep your sanity high, refuse leviathan and anything related to it (no book reading either) and have you occultism at 5.

My last run I had low sanity, includding reading books and accepting leviathan kwnoledge, and even with 5 occultism I didn't get the option. I also drank on this run, so maybe don't drink either.

to get farewell. you need to accept / submit to leviathan, and not shoot bradley and "not tell the truth" when talking about mrs fuller. also also respond with the "sane" option

chose one answer in rlyeh
accepted and saved drake
refused knowledge
spared bradley
saved cat
refused everything except the gun
had some sanity left but not much

and got farewell

How to bid a "Farewell" to Sarah (& Algernon) (an updated version)
İf you want to get this,
Do NOT:
- drink alcohol (it doesn't matter if you drink or not, actually)
- read any unholy book
- choose any answer in R'lyehian (excluding Ch. 11)
- submit & accept Leviathan's knowledge
- take the pills, eat Leviathan's flesh, take the gun, etc
- kill Bradley
- reconstruct Sarah's ritual in Ch. 9)
- reconstruct "The Truth" in the harbourmaster's office (Ch. 12)
- make İrene commit suicide (sanity will be affected at the start of Ch. 10)
- look at the corpses after you healed an individual (Cat, Roy, or the bootlegger [Ch. 12])
