Painkiller Black Edition has no controller support whatsoever. You could probably try Xpadder though. Or you could play Painkiller H&D instead, which has native controller support.

I would however strongly advise against using a controller for this game. You really need the precision of a mouse, not to mention the fact that strafejumping is literally impossible on a controller.

Pinnacle game profiler or others external tools, but remember that is is an fps (fps arena) so mouse+keyboard is the best way.
