Serious Sam is also with gore R-rated.But Painkiller to me yeah it is more darker more Gothic so to speak xd.Serious Sam and Painkiller are amongst my all time favorites with Quake,DooM and many many more
What you can compare this to is Doom 2016 and forward because they took the Painkiller formula and trivialized it to make it more accessible.
Painkiller on the other hand is a adrenaline rush from start to finish, every weapon is useful, tarot cards add uniqueness, boss fights aren't the greatest imo but tarot cards cheese them, killer soundtrack and level design, godly atmosphere and nice enemy variety, it's better than SS in everyway and people who complain about the health system just have a skill issue
