I haven't understand the "bug" that you mean. Video proof or level's name is best.
All I had to do was replay Catacombs and now I start every level with a stake gun. Though I also start every level with every weapon now, even the ones from BooH. odd.
The problem was that I replayed level 1-1 and 1-2 after beating all levels in the chapter. Seems like that reset my starting inventory to just a shotgun and the painkiller. So when I went on to play any chapter 2 level I would only get the shotgun and painkiller. Apparently they didn't think to throw a stakegun into any of the levels after Catacombs. What a strange system to manage weapons.
Also I realized there was a problem when I started Town, the challenge was "only use stakes," I didn't have a stake gun and I couldn't leave the first room without killing an enemy. Impossible.
Essentially the console is disable on Nightmare difficulty. You can open it, but if you try to type in a cheat like 'pkammo' it tells you you have to be playing Daydream or Insomnia difficulty to use cheats.
This problem looks like savegame corruption, but is strange anyway because each level have different start option (about weapons i mean). You can see that by using the editor and loading each levels (need a nocd exe).
ps. you play without fps limit? You use win 10? If yes try to limit them and to run the game in compatibility mode.
