I have Win 8.1, and when I run it in the new graphic card mode, it cuts-off the in-game pop-ups. You cannot see all of your storage slots, for example. In the Market, you cannot navigate between stalls without shutting and re-opening. Out of that mode, the only issue is that the game is choppy. This hasn't been optimized properly.
Your question of "is it worth playing in 2019" is very subjective. It depends on your gaming expectations. I still enjoy playing Torment: Planescape and BG I&II. So, my answer is definitely! Sadly, they offer a VERY limited number of scenarios with this addition, and you cannot access the Marriage Swindler except on very hard assignment mode. I found it to be one of the easiest. They gussied the game up at some point, and this is the result. The original version of this game was CLASSIC!!
As to it working on Win 10, I have Win 8.1. It's choppy without ticking the box for new graphic cards, and ticking that box cuts off the in-game pop-up windows, where you view your inventory, for example.
