The bullet hole in the back of his head is actually a remnant from a previous development idea, where reapers were formed from suicide instead of skeletons. Timur's portrait originally belonged to the first reaper boss, who led the zombies. He'd killed himself with a shotgun, hence the blown-open skull and the missing jaw. I'm not sure what our Reaper's missing jaw means though, since suicide in the release version means they become skeletons, not reapers.
My personal theory (or possibly just headcanon, since I don't have much in the way of evidence) is that reapers are people who died a normal death, but caught the flames' attention in some way, so instead of heading off to the afterlife they're offered a job. Ultimately though, the origins of reapers is left intentionally unanswered at the moment, so it's all just speculation.
